Frame of Mind, LLC, maintains the following Policies and Procedures regarding Polyvagal Theory training(s) offered.

Attendance Policy (applicable when Continuing Education Credits are offered)
State licensing boards approve training(s) for continued ed credit based on the length (hours) of time material is learned, Therefore, participants will only get credit for the number of hours they are physically present in the training. Training sessions are held live, in person, with sign-in sheets for morning and afternoon learning. It is the responsibility of the participant to sign those logs. Additionally, continuing ed credit will not be given without the completion of the course evaluation.

Training Fee’s, Refunds and Cancellation Policy
Training fee’s for set training dates held at Frame of Mind, LLC, in the Anderson Learning Center are clearly listed on the registration page.  If your agency is interested in specialized training, those fees will be determined based on the distance and travel cost for trainers, the size of your organization, and the length of the training desired. Please contact Kelly Perry, LCSW at [email protected] to discuss further.  A completion certificate is provided to participants at the end of the training unless continuing education credits are stated in the course offering. For organizations interested in training with CE’s, there is an additional fee of $1000 per discipline to cover additional time spent with the CE approval process and fees involved in that process.

Frame of Mind, LLC, requires three weeks’ notice in writing to issue a full refund for the previously purchased training. In the case of last-minute emergencies, or unforeseen illness, please contact kellyaperrylcsw@frameofmindtherapy in writing. A full refund will be issued minus a $30.00 processing fee.

Inclement weather policy. Training courses are offered live, in person only; therefore, we ask that participants decide for themselves if driving is safe. Should participants choose to cancel in this case, the same full refund minus $30.00 processing fee will be issued.  In the event of a major weather event, such as a blizzard or record snowfall, participants will be contacted the evening before the training to cancel the event. Participants will be given the opportunity to choose another training date from the course offerings.

Should training providers need to cancel a training event, participants will be offered a full refund, or an opportunity to choose another training date free of charge.

Confidentiality of client information
Frame of Mind, LLC, maintains confidentiality of client information during training sessions. We ask that client information not be discussed or disclosed unless appropriately de-identified.  This includes no mention of name, address, phone, or characteristics that may allow others to identify a particular client.  In the training setting, De-identified mixtures of real-life case examples may be used to provide examples for enhanced learning or facilitate small group exercises, discourse, or role plays. Participants and trainers alike are held to HIPPA laws and ethical codes within their disciplines.

Confidentiality of participant information
Frame of Mind, LLC, ensures the confidentiality of participant(s) personal information in their registration process. Participant’s name, contact information, and financial information is kept confidential via utilization of e-commerce transactions on www.frameofmindtherapy.org. E-commerce companies are required by law to protect customer and merchant information from unauthorized access on the internet. Training courses are held in person, and participants will not be photographed or recorded without their explicit written consent. Should a confidentiality breach occur, Frame of Mind, LLC, will notify affected parties immediately upon discovery of the breach.

ADA accommodation for training participants
Participants who would like to request accommodation must contact the training provider to make such requests in writing three weeks prior to the start of the training event. Requests will be reviewed by the training provider and responded to within one week of receipt. You may contact the training provider, Kelly A. Perry, LCSW, at [email protected]

Participant complaint process
Should a concern arise from a training participant in regard to the trainers or the training materials, we would encourage the individual to email Kelly Perry at [email protected] while Frame of Mind, LLC, does not guarantee a particular outcome, Kelly will review, consider, and consult on the complaint, make any necessary decisions, and respond within two weeks. There is also a course evaluation at the end of each program where participants are encouraged to give feedback on the training and presenters.

In addition, CE Providers should consider developing policies and procedures concerning the matters listed below to further enhance the quality of their business practices and ensure consistency. It is helpful to have policies or guidelines in place before a problem arises.

Training Record Keeping
Frame of Mind, LLC, will maintain information on courses offering NBCC or NASW continuing education credit, for a period of five years. This information will be housed at Frame of Mind, LLC. Participants may obtain copies of these records via written request to [email protected] The following information will be kept:

(1).          A roster identifying all program participants.

(2).          A list of all live event program presenters, including the qualifications of each presenter, and a description of the content presented by that presenter.

(3).          An accurate bibliography/reference list for the training content.

(4).          The program content description.

(5).          The attendee/participant evaluations.

(6).          The training promotional and marketing materials.

(7).            The program agenda and materials distributed to participants, including the learning objectives.
